This is a face-to-face comprehensive clinical assessment conducted by a licensed social worker to determine the individual’s problems, strengths, needs, and abilities to develop a medical and social history plan to achieve their best quality of life.
Counseling services are designed to assist a person or family in identifying and solving personal, social, vocational, interpersonal, and intrapersonal concerns. These may include illnesses and medication management, problem-solving and cognitive skills management, adaptive behaviors, knowledge of mental health, and substance-abuse-related disorders.
This service provides systematic interaction between/with individual and staff and family members directed toward restoring, rebuilding, and enhancing the family unit. This supports the family unit through specific therapeutic interventions and activities to enhance the family roles, relationships, and communication that promote a healthy recovery of/for the individuals involved.
These are case management services that provide rehabilitation, resource coordination, and environmental/housing support that are essential to the clients in restoring themselves to the best functioning level with the greatest degree of life, self-efficiency, and recovery. This is also psychosocial rehab support providing skills needed to return back to a community social setting without fear of others’ pre-judgment. This helps in building self-confidence and interpersonal communication, problem-solving, coping/anger management, and recreational skills.
These are services that are directed toward the support of an individual who is experiencing an abrupt and substantial change in functioning or distress. This service is designed to prevent out-of-community placement or hospitalization.
